Connxus maintains health information as part of its role as a Health Information Exchange (HIE). Individuals have the right to request access to health information maintained by Connxus.
To request access, please complete the Request for Access to Health Information Form or contact us using the information below.
Connxus will provide access to health information maintained within its systems in accordance with applicable law, including the Health Insurance Portability and Accountability Act (HIPAA) and the Texas Health & Safety Code Chapter 181. Connxus will make reasonable efforts to fulfill complete requests within 15 business days.
Connxus does not maintain its own electronic health records system and may not maintain a complete medical record. Health information maintained by Connxus is limited to data received from participating providers and partners. For records not maintained by Connxus, individuals may need to contact their healthcare providers directly. A list of participating providers is available https://connxus.org/partners/.
