We prioritize your privacy and ensure that all information shared through connxus is safeguarded with advanced security measures, including encryption and secure connections. We understand the importance of earning your trust when it comes to protecting your information. Connxus strictly adheres to all state and federal regulations, including HITECH, TX-RAMP, and HIPAA, to maintain the confidentiality of your data.

If your healthcare provider participates in connxus, you are automatically included in the network. Should you choose to opt out or revoke a previous opt-out decision, you can do so using the forms provided below. You may submit the completed forms by mail after having your identity verified by a notary, or you can deliver them directly to your participating healthcare provider. Once your request is processed, connxus will send you a confirmation email for your records.

Opt Out Revocation

If you previously opted out of connxus’ health information exchange and would like to reinstate your participation, you can do so by submitting an Opt-Out Revocation Form. This allows your information to be securely shared with authorized providers and organizations within our system, ensuring seamless coordination of your care.
